# North Star Oregon

> North Star Oregon is a licensed provider of services for individuals with intellectual and developmental disabilities (I/DD), serving families statewide across Oregon. We provide in-home attendant care anywhere in the state and community-based Day Support Activities (DSA) at regional hubs. Headquarters: 232 SE M Street, Grants Pass, Oregon. Phone: 541-208-1106. Services are typically provided at no out-of-pocket cost to families — funded through Oregon's K-Plan (Community First Choice Option) under Medicaid. To start services, families work through their county's Community Developmental Disabilities Program (CDDP) for an eligibility determination, are assigned a Services Coordinator (county) or Personal Agent (brokerage), and build an Individual Support Plan (ISP) that authorizes service hours. North Star Oregon accepts referrals from county Services Coordinators, brokerage Personal Agents, and directly from families.
