Client Intake

Are you or a loved one interested in Attendant Care or DSA services with us? We would love to hear from you! We can answer any questions you may have via email or phone, or we can schedule a meet and greet so that you may have the opportunity to get to know our programs a little better before signing on with us. 

Already have a support provider in mind?

Alternatively, we always love to hear from individuals that already have a support provider or somebody in mind that they would like to work with and would like to sign on with us. If you are an individual that is eligible for Developmental Disability services and you have a friend or family member that you'd like to have as your support provider, please feel free to fill out our client intake form or contact us for more information!

PSWs are always welcome to transfer to us!

PSWs are always welcome to sign on with us if they feel it would be beneficial to work through us. By signing on as a DSP (the Provider Agency equivalent to a PSW), you can expect higher wages, more hands on support, and the potential for additional clients to work with if desired. Your job supporting the individual would remain the same!

How are services billed?

In order to start working with us, we will have your Case Manager add the chosen service to your ISP so that they may be billed under kplan services (kplan services are fully funded through Medicaid). DSA and Attendant Care services come out of your Adult Needs Assessment (ANA) hours. If you do not know how many ANA hours you qualify for, please ask your assigned Case Manager.
