Comprehensive disability support services tailored to individual needs
North Star Oregon provides two core services for individuals with intellectual and developmental disabilities (I/DD): one-on-one in-home attendant care available statewide, and group Day Support Activities at seven hubs across the state. Every plan is person-centered — built around the goals in each individual’s Individual Support Plan (ISP), not around a standard program template. Many of the people we support use both services together: attendant care at home, and DSA outings for community, friendship, and skill-building.

Our day support activities are designed to provide enriching experiences that foster personal growth and community integration. Activities include bowling, art workshops, outdoor adventures, and engagement activities across multiple Oregon locations.

We provide compassionate and comprehensive one-on-one attendant care throughout Oregon, assisting with daily living activities, personal care, and community participation in the comfort of clients' homes.
How In-Home Care WorksWho Is Eligible & How Services Are Funded
Our services are for Oregonians of all ages with intellectual and developmental disabilities, including autism spectrum disorder, cerebral palsy, Down syndrome, fetal alcohol spectrum disorder, and other developmental conditions. To receive services, an individual must be determined eligible for I/DD services through their local Community Developmental Disabilities Program (CDDP). Once eligible, a Services Coordinator or Personal Agent helps create the Individual Support Plan that authorizes hours for attendant care, Day Support Activities, and related supports.
Most services are funded through Oregon’s K-Plan (the Community First Choice State Plan), the state’s primary Medicaid funding mechanism for home and community-based disability services, or through 1915(c) waivers. That means for eligible individuals, services typically come at no out-of-pocket cost to the family. Not sure where to start?
