You know the situation: a client who needs mobility support, personal care, or a higher staffing ratio — and a provider list that keeps saying no. We’ve built North Star Oregon to be the call that works. Higher support needs are evaluated case-by-case, not screened out, and we’ll give you a straight answer on fit and current capacity within one business day.
We work with CDDPs and brokerages across Oregon and know the paperwork on both sides — ISPs, service agreements, ONA service groups, and the OAR rules that govern them.
What you can refer to us
In-Home Attendant Care
One-on-one support statewide — every Oregon county. Personal care, daily living skills, community access. We hire qualified family members as W-2 employees within OAR 411-450-0080 limits, which keeps your documentation clean.
Day Support Activities
Structured community DSA, Monday–Friday 10–3, at hubs in Albany, Corvallis, Eugene, Springfield, Salem, Tangent, and Grants Pass/Medford. We target a 1:5 staff ratio — well below the OAR 411-450-0060 cap of 1:8.
Higher Support Needs
Mobility equipment, bathroom support, high medical or behavioral needs — evaluated case-by-case, and we’re willing to staff situations other agencies won’t. Relief/respite care is also available.
How a referral works

Reach out. Call 541-208-1106 or use the referral form. Tell us the support level, location, and timeline — a real person replies within one business day with a straight answer on fit and capacity.

We meet the family. We coordinate the intake conversation with you and the family, and handle our side of the service agreement and documentation.

Services start. Once it’s on the ISP, we begin — and you keep a direct line to a team member who knows the client, not a call queue.
