Disability Support Agency Serving Grants Pass and Josephine County

Grants Pass is the heart of North Star Oregon's southern Oregon presence. Our office sits at 232 SE M Street, and from there our team supports individuals with intellectual and developmental disabilities across Josephine County and the broader Rogue Valley. Whether you live in Grants Pass, Cave Junction, Murphy, Merlin, or anywhere in between, we show up locally with the same person-centered care we've built across the rest of the state.

Eligibility and service coordination in Josephine County run through the Josephine County Developmental Disability Services program — the local CDDP — reachable at 541-474-5126. If you're already connected with a Services Coordinator or Personal Agent there, we can plug into your existing team. If you're new to the system, we'll walk you through it.

Josephine County families have asked for high-quality, person-centered DSA programming for a long time. We're proud to bring it here.

Disability Support Services in Grants Pass, Oregon

A True Local Presence

We are not parachuting in. North Star Oregon's Grants Pass office is staffed by people who live and work in the Rogue Valley. That means consistent staffing, faster response times, and programming shaped by what southern Oregon actually offers — the Rogue River trails, Riverside Park, downtown Grants Pass, the farmers market, the small businesses that make this region distinctive.

Day Support and Attendant Care — Coordinated

Many families use both our Day Support Activities and in-home attendant care services. We coordinate the two so the support at home and in the community feels like one continuous plan, not two disconnected programs. The same organization, the same standards, the same long-term relationship.

In-Home Attendant Care Services in Grants Pass, Oregon

In-home attendant care provides one-on-one support in the home and community for individuals with I/DD. Our Grants Pass team helps with personal care, daily living tasks, skill-building, and community access. Whether the goal is greater independence at home, support getting to appointments at Asante Three Rivers Medical Center, or help building skills for life in the Rogue Valley, our DSPs are matched to the people they serve and stay for the long haul.

Our attendant care workers come to you anywhere in Josephine County — you don't need to travel to a facility. The point is meeting people in the lives they actually live.

  • Personal care and daily living support
  • Skill building toward greater independence
  • Community access and social engagement
  • Assistance with medical appointments and errands
  • Companionship and consistent, familiar support

Day Support Activities in Grants Pass, Oregon

Our Rogue Valley Day Support Activities (DSA) program is based in Grants Pass and serves participants across Josephine and Jackson Counties. DSA is a community-based program funded through Oregon Medicaid (the K Plan and 1915(c) waivers) for adults with I/DD eligible through ODDS.

A typical week mixes outdoor time on Rogue River trails, art and music at local studios, visits to the Grants Pass farmers market and downtown, volunteer shifts at community organizations, and skill-building tied to each participant's ISP goals. Group sizes stay small, staffing stays consistent, and the schedule flexes around the people in the room.

Visit our dedicated Rogue Valley page for a deeper look at the program, including who it's for and how to enroll.

About North Star Oregon

North Star Oregon serves individuals with intellectual and developmental disabilities throughout Oregon. Our Grants Pass office anchors our southern Oregon work, with both DSA and in-home attendant care delivered by a team that knows the Rogue Valley firsthand. We are a direct-employment provider — our Direct Support Professionals are W-2 employees with training, supervision, and benefits, not 1099 contractors. That distinction shows up in continuity of care and in the depth of the relationships staff build with the people they support.
